Youth FIRST Athletics · Parental Consent and Acknowledgment

Effective date: September 27, 2026

This page describes what a parent or guardian agrees to, and how, when an athlete joins Youth FIRST Athletics. There is no separate form to sign here: each item below is a step in the app, and the app keeps the record of it.

1. Your parent account

You create your own account with Coach's join code and your email, and Coach approves it before it can be used. Everything about an athlete under 13 is managed from this account: they never get a login. An athlete 13 to 17 can have their own login only after your account exists and you approve the link from it.

2. The signed agreement

When you add an athlete, or in the guardian step of a teen's own signup, you sign the Assumption of Risk, Waiver of Liability, and Parental Consent Agreement by typing your full name. It covers the risks of training, the release of liability, medical treatment authorization and program policies, and in its section 8A, "Your Family's Data," it is your consent, on your athlete's behalf, to the data practices the Privacy Notice describes. The app stores your typed signature with the date and the agreement's version, so the record always shows which text you signed, and a copy of the agreement can be downloaded from the same screen.

The same step collects what Coach needs to keep your athlete safe: your relationship to the athlete and your phone, your address, an emergency contact, the physician's name and phone, medical notes and allergies, and any injuries to report. You can update them later from your account menu.

3. The photo and video question

Section 6 of the agreement asks whether Coach may photograph or video your athlete during sessions and use those clips for marketing, social media, and promotion, without compensation. Either answer is fine, and training is not affected. Afterwards only a parent or guardian can change the answer, at any time, under Data & Privacy; Coach can ask, but never answer for you. What each answer does is in the photo and video release.

4. The Privacy Notice

At your first login, and again whenever a new version is published, the app shows you the Privacy Notice and continues only once you accept it (or log out). Each acceptance is recorded with its version and time, and every version you accepted stays available under Data & Privacy.

5. The yearly concussion form

Florida asks a parent or guardian to sign a concussion form once a year before training. Until it is signed, your athlete cannot start a workout or book a session in the app; signing takes about a minute and covers the next twelve months.

6. Separate yeses, each asked on its own

7. Florida Parents' Bill of Rights

We recognize your rights under Florida's Parents' Bill of Rights (Fla. Stat. § 1014.04) to direct the upbringing, education and health care of your minor child, including access to and control over your child's records with us. There is no checkbox for this in the app; it is how we run the program, and nothing in the app overrides it.

Changing your mind

You can change any of these choices at any time, delete your athlete's collected data, or ask to close the account; see Data Retention & Deletion.
